Only on this phone. Remove it any time with Reset, or with "Forget all of it" on your account page. The lines on the card, and the note to the public on its back, are the card printed in Appendix 5 of Madhumeha Manohara, by Dr Shashikiran Umakanth. The card is always in both Kannada and English, whichever language you read this page in, because the person who finds you may read either.

The printed card ಮುದ್ರಿಸಿದ ಚೀಟಿ

It prints on an ordinary A4 sheet as two cards, each the size of a bank card when folded. Print at 100 per cent, not "fit to page". Cut along the dashed line and fold along the dotted line in the middle. Keep one in your wallet and one in your bag. When a medicine changes, make the card again.

The lock-screen picture ಲಾಕ್ ಸ್ಕ್ರೀನ್ ಚಿತ್ರ

A phone is often the one thing a person found unwell has on them, and its lock screen can be read without unlocking it. The picture leaves the top of the screen dark and empty, where the clock sits. It carries no QR code.

What happens to what you type ನೀವು ಬರೆದದ್ದು ಏನಾಗುತ್ತದೆ

The card and the picture are made inside your browser, on this phone. Nothing goes to this site's server, or anywhere else. If you tick "Keep on this phone", your entries are stored on this phone only, until you press Reset or "Forget all of it". Without the tick, nothing is kept after you close the page.
